• Schedule:  Day  / Full-time, Part-time, or Weekends Only 
• Shift:   PM: 3:00 pm – 11:00 pm  with every other weekend rotation

Community Care is an SNF with 204 beds and multiple levels of care, including a large behavioral patient population (ages 21 and up) within the center. 

• Revised clinical initiatives include a new Physiatrist, Behavioral Health activity-based programming, and a new Vohra Wound program coming soon!  
• We are located near W 43 ST & State Street, just off 90/94 and a few blocks from the train station.

Certified Nursing Assistant (CNA): officially we title this job a Certified Nursing Assistant, but in other centers, it may be called a CNA or Certified Nurse Aide.

Certified Nursing Assistant (CNA): Sometimes called a Certified Nurse (or Nurse’s) Assistant or Aide.

What we need from you:

• Certified Nursing Assistant (CNA) certification in IL
• Prior experience in a hospital or short-term rehab unit within a skilled nursing facility is ideal, but new grads are welcome
• Electronic medical records experience (Point Click Care) is helpful
• We follow the CMS & IDPH rules for the Covid-19 vaccine.  You should be fully vaccinated or you will be required to submit an exemption.  Details will be discussed during your interview.
